CONSENT TO THE PROCESSING OF PERSONAL AND SENSITIVE DATA

I declare that I have been informed that:

1) the personal data disclosed to Studio Viale von der Goltz will be processed using an integrated computerised system and hard copy sheets, telephone books and agendas; it will only be disclosed to other subjects for the purpose of fulfilling the appointment made by the event organiser, for which it has been conferred;
2) conferral of personal data is required in order to adhere to the event for which it has been conferred;
3) personal data will be processed and potentially disclosed to the third parties specified under point 1, purely for the purpose of managing the relationship;
4) data processed using computerised storage devices will be stored in compliance with the provisions of Italian Legislative Decree no. 196/2003 and, in any case, by taking suitable security measures to protect the archives, which will vary according to the technological evolution of the corresponding hardware and software;
5) access to personal data is granted, under the scope of the specific competences of each, including administrative and secretarial duties, to the signatory owner and to employees and collaborators of Studio Viale von der Goltz;
6) the data controller and data supervisor is the signatory owner of Studio Viale von der Goltz, to this end with domicile elected at Via Assarotti 31/9 in Genoa, where the business is based;
7) data subject rights with regards to data processing are those laid down by Article 7 of Italian Legislative Decree no. 196 of 30 June 2003 and transcribed herewith:

“A data subject shall have the right to be informed of:
a) the source of the personal data;
b) the purposes and methods of the processing;
c) the logic applied to the processing, if carried out with the help of electronic means;
d) the identification data concerning the data controller, data processors and the representative designated as per article 5, subsection 2;
e) the entities or categories of entity to whom or which the personal data may be communicated and who or which may get to know said data in their capacity as designated representative in the State’s territory, data processors or persons in charge of the processing.

A data subject shall have the right to obtain:
a) updating, rectification or, where interested therein, integration of the data;
b) erasure, anonymisation or blocking of the data that has been processed unlawfully, including data whose retention is unnecessary for the purposes for which it has been collected or subsequently processed;
c) certification to the effect that the operations as per letters a) and b) have been notified, as also related to their contents, to the entities to whom or which the data was communicated or disclosed, unless this requirement proves impossible or involves a manifestly disproportionate effort compared with the right that is to be protected.

A data subject shall have the right to object, in whole or in part:
a) on legitimate grounds, to the processing of personal data concerning him/her, even though this is relevant to the purpose of the collection;
b) to the processing of personal data concerning him/her, where it is carried out for the purpose of sending advertising materials or direct selling, or for the performance of market or commercial communication surveys.”

I confirm that I have read the above disclosure and specifically and expressly authorise the processing and disclosure of the personal data, pursuant to point 1, under the scope of the purposes and in the ways described under points 1, 3 and 5.
